Title:- RN Case Manager – Home Health

Location:- White River Junction, VT

Shift:- Day shift - 8:00am - 4:30pm, Monday - Friday w/rotating weekends

Contract:- 16 weeks of job.

Minimum Qualifications:

Must be a licensed Registered Nurse in Vermont and New Hampshire or hold a compact license.
Graduate of an accredited school of nursing.
Medical/Surgical nursing and/or home health care experience.
